Licensed Vocational Nurse
The LVN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LVN assigned in the in-patient care areas demonstrates active collaboration and communication with RN, Patient, Primary Care Provider, and other care team members. It is also imperative that the LVN establish a positive rapport with family and caregiver.
