The Home Based Primary Care (HBPC) LPN/LVN functions as a team member under the clinical guidance of a Registered nurse. The LPN/LVN is self-directed and functions in a highly independent role within the home care setting; participates in interdisciplinary care planning. Most of the physical work is done in Portland, OR Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
