The PACT Primary Care Clinics: Licensed Practical Nurses (LPNs) work under the direct supervision of Registered Nurses (RNs) and are part of the Patient Aligned Care Team with a focus on patient centered care delivery. The LPN is responsible for actively participating as part of a high functioning team in meeting PACT initiatives and outcomes for a panel of 1,200 patients. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
