The GeriPact Clinic: Licensed Practical Nurses (LPNs) work under the directions of Registered Nurses (RNs) and are part of the Patient Care Aligned Team with a focus on patient centered care delivery. The LPN is responsible for actively participating as part of a high functioning team in meeting PACT initiatives and outcomes.
