The Salem Veterans Affairs Medical Center (VAMC) is seeking a Licensed Practical Nurses (LPN) in Ambulatory Care as a part of the Patient Aligned Care Team (PACT) in the primary care clinics. The LPN participates in interventions that promote the Veteran attaining and maintaining maximum functionality through health promotion, health education and coaching on wellness, disease prevention, and chronic care management. This is a Term position Not-to-Exceed (NTE) 1.5 years. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
