The Licensed Practical Nurse (LPN) works collaboratively in an interdisciplinary coordinated care delivery model (i.e., patient aligned care team). The incumbent is primarily responsible for patient and clinic flow, efficiency of the clinic, customer service, and teamwork. The LPN develops/maintains effective and efficient communication with the patient, interdisciplinary team, VA medical centers, and other agencies.
