The PACT LPN is responsible for assisting in the managing and optimizing self-care management of primary care patients. The PACT LPN demonstrates outstanding collaboration and communication with the Core Team (Patient, Primary Care Provider, RN Care Manager, and Clerk) and Expanded Team (family/caregiver), internal and community-based services involved in providing care to the patient, to provide timely access, comprehensive services, and coordination to optimize the patient's care.
